OBSERVED NO PEST CONTROL LOG BOOK PROVIDED ON THE PREMISES FROM THE CURRENT PEST CONTROL COMPANY. MANAGEMENT INSTRUCTED TO PROVIDE A PEST CONTROL LOG BOOK ON THE PREMISES WITH THE COMPANY'S IDPH CERTIFICATION NUMBER AND ALL RECENT PEST INSPECTION REPORTS. PRIORITY FOUNDATION VIOLATION 7-38-020(C). CITATION ISSUED.
